THERAPIST’S RESOURCE KIT *** Therapist Resource Kit temporarily unavailable while it is being updated. *** Revised and updated in 2004, the Kit is a collection of forms and handouts to be used in clinical practice. The forms can be used as they appear or can be copied onto your own letterhead/stationery. There are over 50 forms or handouts included in the Kit.

A sampling of what you will find:

• Client Hand Outs (A Patient’s Bill of Rights, Choosing a Therapist, What About Brief Therapy?, and more);
• Therapist’s Information (Sample Outpatient Tx Report, Psychotropic Medications, Referral Tracking, and more);
